2024-2025 Head Start Registration Process

2024-2025 Head Start/Early Head Start Registration ProcessIf you received an email stating your child was selected into the Head Start/Early Head Start program for the 2024-2025 school year, please do the following:
Step 1. Download the Required Head Start/Early Head Start Health AND Family Enrollment Forms below and save them to your computer.

Health Enrollment Forms CreoleComplete the forms and sign them using Adobe Fill & Sign. For instructions on how to sign the Enrollment Forms electronically using Adobe fill & Sign, please click one of the links below:
Step 2. Please contact headstart@browardschools.com to obtain your individual link to upload your documents to Child Plus. ***Please DO NOT email your documents to the Office of Head Start.**
Upload a copy of the Required Enrollment Forms from Step 1 and Required Medical Records to Child Plus using your individual link.
Required Medical Records:
- Original (up-to-date) Florida Certificate of Immunization (DH Form 680)
- Physical Exam Form (within last year). This includes:
- Lead Test Results
- Anemia Blood Testing (Hct or Hgb)
- Height & Weight
- Blood Pressure
- Vision & Hearing
- Proof of Dental Exam (New Requirement). Click to complete the Dental Form: (Dental Form)
Step 3. Please visit your child's assigned school to complete the registration process. You will need to bring the following documents with you when you visit your child's school:
- Proof of Age (child’s ORIGINAL birth certificate or passport)
- Parent/Guardian photo identification
- Proof of Guardianship (if applicable)
- Proof of Broward County Residence (Please see list of acceptable proof of residence documents below)
- ORIGINAL mandatory medical records (see above in Step 2)
Note: If you share the housing of another person who owns/rents the home, both the registering parent and owner/renter of the residence complete a notarized Affidavit of Shared Residence Form.

If you OWN or RENT your residence: Submit one document from both columns A and B:
COLUMN A COLUMN B - Property tax bill
- Homestead exemption card
- Deed
- Mortgage statement
- Home purchase contract
- Notarized lease agreement
- Utility bill (i.e., electric, water, waste)
- Telephone or cellular phone bill
- Verification of Tenancy letter from the homeowners or condominium association
- Declaration of Domicile Form from the County Records Department
- Florida drivers license
- Florida identification card
- Automobile registration
- Automobile insurance
- Credit card statement
- Two consecutive bank account statements
- U.S. Postal Service confirmation of address change request
