Instructions For Employees Requesting Accommodation.
* Complete all requested information. The fields marked with an * are required.
* After submitting this request, email your supervisor/principal, to inform him/her that you are requesting an accommodation.
* If you have your physician/health care specialist documents, you can upload them accompanied with this request. If you do not have the documents, you will be required to upload them electronically within 10 days of submitting this request via email to: eeo@browardschools.com. All ADA applications will be considered incomplete without a medical letter on letterhead from the employee's treating physician. The letter needs to specify the employee's medical condition and their limitations based on their impairment. It is the responsibility of the employee to submit their medical documentation to the EEO/ADA Compliance Department for ADA accommodation.
* Once received, the EEO/ADA Compliance Department will contact your immediate Supervisor for input. If necessary, a meeting with you, your supervisor, and the EEO/ADA Department will be scheduled.
* Within 30 days of receiving all relevant documents, including information from your physician or health care specialist, the EEO/ADA Compliance Department will inform you and the Principal/Department Head or immediate Supervisor of its findings and recommendation(s).
