    Once you've completed ALL the required documents and received a sports physical, the documents should be returned to the Head Coach of the sport in which you intend to play in the 2021-2022 school year. 

    Students and parents should read EACH document carefully and sign EACH document where applicable.  You will need proof of insurance dated within the year of 2021.  If you do not have insurance that indicates you are covered for the year of 2021, please download a letter of coverage that shows you are still medically covered in case of injury. 

    All documents must be completed, signed, and returned to the Head Coach of your sport of interest for approval prior to participating in any athletic activities involving Dillard High School 6-12.  Any and all questions should be directed to the Head Coach of the sport of interest or the Athletic Director
