In order to register your child, kindly complete, print, and bring the following documents with you to your registration appointment:

• English Registration Packet / Paquete de Registro en Español
• English Shared Housing Form / Vivienda Compartida en Español
  • This form is for school age children(ren) who reside with their parent/guardian with another person whose residential address is in our boundaries.  This form is on an as needed basis and does not need to be included in every packet. 
• Request of Records Form 
  • This form is for students that attended another school (private or public in or out of state/country) the prior year. 
• Home Language Survey
• English Emergency Card / Tarjeta de Emergencia en Español 
• Health Survey

Upon completion of the above, please review the Registration Checklist to ensure your packet is complete.
