  • "The parent opt-out window is February 12-16 and is for NEW students only. **Any student who was opted-out for the fall survey will be opted out for the spring survey. Students do not need to complete another opt-out form. If your student began at Park Lakes after the last survey on October 23rd and you DO NOT want them to participate, please complete the opt out form.
