In order for you to receive waivers this year for the ACT and SAT tests as well as College Application waivers, the following two forms must be completed.   

This needs to be done even though you automatically receive F/R lunch.   

Step 2 is mandatory for everyone to see if you qualify for waivers.  

Please make sure that all boxes are clicked.

NEW REGISTRATION PROCESS: 

STEP 1: Apply for Free/Reduced Lunch at https://www.myschoolapps.com/ 

Click on Broward schools Household Income Survey – complete form 

STEP 2: Complete the Meal Benefits Disclosure Form at

*Doing this will give you access to SAT/ACT fee waivers, unlimited SAT score sends, and college application fee waivers.
